This health awareness site provides you with an opportunity to take a health risk assessment survey. The survey gives you a way to find out about your health risks and ways to improve your health.

Taking a survey on your health is not a substitute for medical exams with your primary care provider. This survey obtains your risk, in general terms. It does not medically evaluate, diagnose, or provide suggested treatment for any medical condition. Please read the terms of use for this website.

After you submit your survey, you will receive a report that will help you understand:

• Health behaviors or conditions that may lead to future health problems
• Ways to improve your health
• Resources for more information and assistance

In general, employers use health status awareness surveys to understand their employees’ health. Wellness programs can then be designed to meet the areas of greatest health risk in their organization. A wellness program using preventive health measures may improve health status and lower health care costs, including insurance premiums.
