Welcome!
Our Customer Service is offered to persons with survey navigation questions
or website problems. We also welcome initial requests from health plans,
employers, and others seeking more information about the health status awareness
survey. An agency account number can be set up to gather health data on your employees.
The data will be provided to you in aggregate.
 In using this service, please email our staff at
assessmentcenter@spokanecounty.org with the following information
in as much detail as possible:
- The nature of the problem, including any on-screen messages that you received
- The date and time that the problem occurred
- Your phone number, complete with area code
- An email address that you check regularly
We will check out the problem with the website and get back to you in a reasonable
amount of time. We cannot answer questions that you may have like, "What do
my health risk results mean?" or "Should I report these results to my
doctor?" We cannot give medical advice, so remember to focus your questions
on issues with the website.
