How to Fill Out the Survey

Use your mouse to answer the questions in the survey. You can tab between the questions, if desired. If you need to change answers, click on the new answer.

The health areas within this survey have been broken down into:
• general health
• health care
• oral health
• weight control
• physical activity
• nutrition
• tobacco and alcohol use
• preventive measures
• health information
• demographics

If you are filling out the survey as an employee at a company and have an agency code, type in the 4-digit code at the top of the survey. Otherwise, leave blank.

After you have finished the survey, click on the submit button at the bottom of the page and you are done. Your individual report will be created for you to print out a copy.
